RMA Product Request
IMPORTANT - Please Read ALL Non-warranty product returns will be subject to an $85.00 bench fee for diagnostic purposes. This fee applies to each individual returned item. However in many cases, multiple inspections may be completed during the initial one-hour diagnosis which would NOT increase the evaluation fee. If the repair(s) are approved, the $85.00 will be applied toward the repair(s) cost. In cases where repairs are not authorized after diagnosis, the customer will be responsible for the evaluation fee and will be given the option of having SCAN scrap the unit or return the unit to the customer with appropriate shipping charges associated. Should you need to return a product for service, please follow these instructions:
1) Fill in all the appropriate blanks on the following RMA form.
2) Upon proper completion of this form, click the SUBMIT button located below. Otherwise choose RESET and input information again.
3) Include in the box a copy of the returned RMA approval form/correspondence along with a SAMPLE document for testing with your shipment.
4) Write the RMA number boldly on the side or top of the box.
